The therapist processes personal information to enable the provision of complementary therapy services to clients: to maintain accounts & records, promote services & availability and arrange appointments.
Only data provided by clients during their initial contact and subsequent appointments or conversations will be collected.
Data will be limited to contact information, medical and health information, lifestyle information including living arrangements where they may contribute to medical information, and other specific information where it is relevant to the health care/ treatment plan of the client.
Data will not be used for marketing purposes and will only be used to contact the client in the following circumstances:
- For the discussion of treatment outcomes and agreement of treatment plans
- For the arrangement/re-arrangement of appointments
- To inform clients if the therapist will be unavailable for a short period of time such that clients are aware any messages may not be received by the therapist
- To occasionally send best wishes
- To request feedback on the treatment they have received.
Data will not be shared with a third party unless prior consent has been received and only in relation to when joint healthcare with another practitioner is being considered.
A client may request to see any data that is held.
A client may request that all data held is deleted. When this occurs, data stored for the purposes of the therapist’s accounts and records will be deleted as appropriate with the exception of clinical notes, which the therapist has a legitimate interest in retaining for 10 years as advised by therapy insurance providers and professional associations (Balens, Federation of Holisitic Therapists, Bowen Therapist Professional Association and the Complementary and Natural Healthcare Council).
Data will be stored securely, as set out in the therapist’s Data Security Information document, a copy of which is available on request by emailing email@example.com
